Access the Patient Portal for Health Information

MyDoorCountyHealth.org is a free, online tool to help patients manage their health. Patients will have secure access to health information, as well as that of their minor children and other family members who designate access. The patient portal allows patients access to:

Health Information

Request Records to be Sent to Third Party

The Health Information Management Department of Door County Medical Center is happy to assist patients in arranging for a copy of their health records to be sent to insurance companies, Workers Compensation carriers, disability agencies, attorneys, etc. Federal and state law requires a more detailed authorization for this type of disclosure.

Request a Copy of Health Records for Personal Use

The Health Information Management Department of Door County Medical Center retains patient health records for as long as the patient is active. Patient records that are inactive after a period of ten years are destroyed in a confidential manner. In 2011 Door County Medical Center transitioned to electronic health records. Paper records have been maintained on active patients.

In addition, limited patient health information is available on the Door County Medical Center Patient Portal. Medications, diagnoses, laboratory results, and diagnostic studies are available on-line by signing up for the Portal by contacting Door County Medical Center at 920-746-3509.

Immunizations/vaccination records may be found at the Wisconsin Immunization Registry (WIR) which is a computerized internet database application that was developed to record and track immunization dates of Wisconsin children and adults.

Another option for fast access to your health information is to sign up for the Patient Portal. Your laboratory and diagnostic imaging results are posted to the Patient Portal 24 hours after completion.

The Health Information Management Department of Door County Medical Center is happy to facilitate continuing patient care by transferring health records to other healthcare providers.

Request Records to be Sent to Your Door County Medical Center Provider

Your Door County Medical Center (DCMC) provider may benefit from having access to copies of your health records that were created by other healthcare facilities. If your DCMC provider has requested that you obtain copies of these records for your DCMC health record, you have two options for making this happen.

1. Contact the healthcare facility by phone and ask that a copy of your records be forwarded to:

2. Download the DCMC authorization form, complete the highlighted yellow fields, and mail or fax it to the healthcare facility. Upon receipt of the authorization, the healthcare facility should forward the requested copies to your DCMC provider.
