In order to accommodate increased testing of COVID-19, Door County Medical Center has established an online COVID-19 Screening Form.
Starting Tuesday, October 13, 2020, patients who are concerned about exposure to or symptoms of COVID-19 should visit dcmedical.org/covid19 to fill out an online form. Upon submission, patients will be contacted by Door County Medical Center staff as soon as possible Monday-Friday 8 a.m. to 4:30 p.m. to discuss their case and potentially schedule a COVID-19 test.
If you are experiencing symptoms related to COVID-19, visit www.dcmedical.org/covid19 and fill out the online questionnaire. The DCMC COVID-19 hotline is still available, however most callers requesting testing will be asked to complete the online form.
Symptoms include:
- Fever/chills
- Headache
- Sore throat
- Diarrhea
- Chest tightness
- Shortness of breath
- Any respiratory illness
- Body aches
- Nausea/vomiting
- Fatigue
- Cough
- Alteration in smell or taste
- Nasal congestion
- Runny nose
