Advance Directive Presentations

What is your wish?

Overview

HOW WILL WE KNOW? WHAT IF SOMETHING HAPPENS?

Be prepared – fill your Advance Directive out today!

At Door County Medical Center, we value your right to consent to medical treatment before it is provided. While our medical professionals are trained to provide you with solutions to your healthcare concerns, we know that those solutions need to be evaluated based on your own healthcare goals and personal values. Some of the most difficult decisions you and your loved ones will have to make include the decisions related to end-of-life medical care. Join Door County Medical Center Lead Chaplain, John Hauser and Social Worker, Erin Szakala as they discuss how to write down your healthcare wishes in an advance directive.

Erin and John will be available between and immediately following the presentations to answer questions and witness documents. Learn more about advance directives.
