There are numerous causes of memory loss, changes in thinking, or behavior.  Our Memory Care program offers accessible, affordable and efficient means of diagnosis and support for individuals, families and friends coping with memory concerns.

Free Evaluation & Support
Anyone can contact our memory care specialist for free evaluation, often conducted in the home or in one of our Door and Kewaunee County locations.
Our certified care manager is also available to consult with you with regard to behavioral concerns, future planning, or supportive resources.

Team Approach to Diagnosis
Our memory clinic team of Physician, Geriatric Nurse Practitioner, Occupational Therapist, Nurse, and Certified Care Manager, partner with the Wisconsin Alzheimer’s Institute of UW Madison to diagnose dementia-related disorders, provide evidence-based therapies, and connect you to current research opportunities. Comprehensive testing and diagnostic consultation is completed within one 2-hour visit to our clinic in Sturgeon Bay.

Opportunities to Do Something About It!
Our memory care program features a number of opportunities for individualized therapy, group workshops, support opportunities, and educational activities. We also partner with community resource providers to maximize the circle of support so you can go on living with security and confidence.
