Insurance Information and Price Transparency

Patient Financial Services will submit your claim to your insurance carrier, provided you have provided us with the name and policy number of your insurance. Deductibles, co-insurance, and non-covered balances not paid by your insurance will be your responsibility and receipt is expected within 30 days. If you cannot pay or have questions regarding your bill, please contact Customer Service at 920-746-3502.

Financial Assistance

At Door County Medical Center, we believe that excellent medical care should be available to all individuals, regardless of their financial situation. We offer financial assistance to those who qualify. Our dedicated financial counselors are available to assist you in understanding our program and completing an application.

Eligibility Requirements

In order to determine the level of financial assistance available we look at family size, income and assets. Recent changes to your financial state are also taken into consideration. Our general guidelines indicate which level you may be eligible for.

Financial Assistance is available to the insured, underinsured as well as those with health insurance exemptions.

How Can I Apply?

In order to apply for financial assistance you generally need to complete our Financial Assistance Application and provide supporting documentation as detailed in our Financial Assistance Policy.

If you need assistance completing the Financial Assistance Application, our financial counselors are available to help you. Please call 920-743-5566 to speak with a financial counselor. Applications are available at all Door County Medical Center registration desks and Financial Counselors.
